De acordo com o artigo 8º da Resolução CFO-157 do Conselho Federal de Odontologia, a utilização de células humanas, incluindo células-tronco e seus derivados, em procedimentos clínicos em desacordo com a legislação, bem como o oferecimento de procedimento odontológico sem o devido reconhecimento científico e terapêutico pelos órgãos competentes, configura infração ética.

Entretanto, a resolução CFO-158, da mesma entidade, regulamenta o uso de agregados plaquetários autólogos (plasma rico em plaquetas e fibrina rica em plaquetas). Apesar de não fazer parte do mesmo conceito de terapias com uso de células-tronco, traz grandes benefícios em procedimentos odontológicos, principalmente os cirúrgicos. Vamos entender melhor a seguir.

O que são agregados plaquetários autólogos e quais os benefícios para a Odontologia?

Os agregados plaquetários podem ser definidos como o Plasma Rico em Plaquetas (PRP) e a Fibrina Rica em Plaquetas (PRF). São frações do sangue que contém componentes plaquetários. A regulamentação do CFO permite a manipulação de agregados plaquetários apenas para uso autólogo, ou seja, quando se utiliza os componentes do sangue do próprio paciente no qual foi feita a coleta. Portanto, sem fins transfusionais.

O dentista realiza a punção venosa, que é a retirada de sangue do paciente e coloca em uma centrífuga para separação mecânica de agregados plaquetários autólogos. A parte plaquetária fica com uma consistência gelatinosa para ser aplicado no local cirúrgico. Tanto o plasma rico em plaquetas (PRP), quanto o plasma rico em fibrinas (PRF) são úteis em procedimentos cirúrgicos, especialmente em cirurgias de enxerto ósseo.

O PRP é, por exemplo, utilizado para acelerar o processo cicatricial de cirurgias. Ele possui componentes chamados fatores de crescimento, que naturalmente fazem parte do processo de cicatrização de feridas e regeneração de tecidos no corpo humano. A diferença, é que no PRP a concentração de plaquetas é muito mais alta e dessa maneira pode ser aplicada diretamente no local da cirurgia para acelerar a cicatrização.

O PRF também é obtido via centrifugação do sangue, porém este concentra leucócitos e plaquetas, fibrina e células-tronco. Esse concentrado é moldado no local cirúrgico, especialmente em enxertos ósseos, liberando fatores de crescimento para acelerar a cicatrização e estimular a regeneração tecidual, principalmente na regeneração óssea em enxertos.

Com relação a substituição de um dente perdido, enquanto as pesquisas com células tronco continuam a avançar, a melhor alternativa continua sendo o implante dentario. Os implantes dentarios são alternativas excelentes e seguras para ocupar o lugar da perda dentaria.
