Full Arch Dental Implants – All-on-4 – All-on-6 – All-on-X

Full Arch Dental Implants is a Full Mouth Reconstruction treatment, that can be done in the mandible (lower arch) and maxilla (upper arch), although there are important anatomical differences that interfere with the planning of dental implant surgery. Fixed prostheses on dental implants can be made of Zirconia, Pure Zirconia, Porcelain(metal-ceramic), as well as Resin.

This treatment is also known as Fixed Full-Arch Prosthesis or Implant-Supported Denture. This type of implant treatment for all teeth, allows patients to regain their smile and chew.

Dental implant surgery is currently a fairly straightforward operation, carried out in an office or hospital environment, with hospitalization for the patient’s comfort. Implants make it possible to replace missing teeth even in large areas of the arch, including in patients with all their teeth compromised by periodontal disease.
They can also be used in patients with bone loss, who may need bone grafting before or during the installation of the dental implant.

It is important to note that there are currently different types of dental implants, and the most modern ones, cone morse implants, allow for faster healing.

Full Arch Dental Implants . Implant-Supported Denture step by step

With regard to the end result, the aim is to obtain fixed dental prostheses that will remain supported on the implants. This will allow the patient to regain their masticatory function and aesthetics.

Of course, as Fixed Full-Arch Prosthesis are screwed onto dental implants, the result is a very efficient dental prosthesis for chewing, as well as excellent aesthetics and comfort.

It is therefore a much better solution than a removable prosthesis, as it provides excellent fixation and stability. In the upper arch, the full arch dental implants treatment has the added advantage of completely uncovering the palate (roof of the mouth), which improves patient comfort.

Unlike removable prostheses (dentures), which cover the roof of the mouth to allow them to be fixed and make it difficult to perceive the taste of food, All-on-X prostheses with implants are thin and occupy only the region of the dentition, leaving the rest of the mouth free, similar to the situation found when there are natural teeth. Full Arch Dental Implants .Examinations and planning

After radiographic examinations and a CT scan of the jaws, the dental implant specialist will be able to analyze the ideal planning for the case and thus indicate bone complements such as grafts to better prepare the bone if necessary.

Full Arch Dental Implants With or Without Bone Graft ?

If bone grafting is necessary, don’t be alarmed – it’s very common. Of course, dental grafting for full archa implants is not necessarily complicated, invasive or large. On the contrary, most of the time the bone graft for a full arch implant is simple, small and restricted to the position of the implant.

Metal-ceramic crowns and prostheses – traditional porcelain

Metal-ceramic crowns and prostheses are a classic option for people who have lost one or more teeth, to fully or partially cover a damaged crown or to improve the appearance, shape or alignment of teeth. Porcelain dental crowns are the popular name for metal-ceramic crowns, but today there are other types of porcelain for dental crowns.

Advantages of metal-ceramic prostheses

The positive points of metal-ceramic crown technology are their strength, high level of durability and appearance. With the new generations of different porcelain formulations, they look very similar to natural teeth. In their final finish, porcelain prostheses are given a special shine so that they don’t darken under the effect of black light.

How crowns are made from this material

A metal-ceramic crown consists of an internal metal structure, which is then veneered with porcelain (ceramic).

Metal-ceramic crowns have a great ability to blend in with the natural coloring of neighboring teeth. They can be used, for example, to replace a large restoration, protect a weakened tooth, make prostheses for one or more teeth, restore a dental implant or a full prosthesis to replace all the teeth in an arch. In short, when making a prosthesis, the patient and the dentist will decide on the best option for each person.

If it’s a metal-ceramic prosthesis, the specialist will choose the correct color so that it matches your natural teeth. Before your permanent porcelain crown is made, you will receive a temporary dental crown. In some cases where natural teeth are reconstructed using porcelain crowns, the root canal-treated teeth will need an internal reinforcement pin called a dental core.

Disadvantages of the metal-ceramic crown

The structural metal of the metal-ceramic is the main strength of the metal+porcelain prosthesis (resistance), but it is also its main disadvantage. The internal metal of the prosthesis or dental crown can make the work less aesthetic because:

  • A dark spot may appear on the tooth due to the translucency of the metal.
  • Dentures can turn gray
  • A metal-ceramic porcelain crown can show metal on its edge, leaving the gums dark
  • The porcelain of dentures can detach one day, leaving the bare metal exposed
  • They are difficult to repair in the event of a fracture
  • They are less living than natural teeth or more modern prostheses such as dental zirconia.

What if I don’t want metal in my mouth?

Another option to metal ceramics can be metal-free porcelain, which is very suitable for front teeth or for people who are allergic to metals. Nowadays there are several types of metal-free crowns, including computer-made crowns, in which the molds are not made with putty but with digitized molds.

It is well known that 3D printers have great versatility for making various objects. The good news is that 3D printing is now a reality in dentistry and is increasingly transforming the way treatments are carried out.

Cerec Sirona System and 3Shape Trios

They are Cad/Cam systems capable of building various types of parts, for example prostheses, bridges, highly aesthetic crowns, dental restorations, ultra-thin dental contact lenses. They work in three main phases:

1 – Intraoral scanner: a high-precision camera that captures images of the inside of the patient’s mouth. It takes a digital impression, thus eliminating the need to use putty. We use two of the main intraoral scanners on the market, the Omnicam and the Trios 3Shape.

2 – Computer: we carry out planning with two of the main systems, Cerec 3D and 3Shape Dental System. The images captured by the intraoral scanner are used to design and simulate three-dimensional prosthetic pieces with shapes and shades according to each patient’s needs.

3 – 3D printers: a milling unit with a robot that sculpts the prosthetic part out of a rough ceramic block. The part is then taken to a kiln for the ceramic sintering process to give it strength.

Ceramill Motion 2 milling machine

It uses Cad/Cam technology to digitally produce dental prostheses in different materials such as zirconia, wax, metal alloy, resin, glass-ceramic and lithium disilicate. It has exclusive software for projecting and simulating prostheses.

Once the design is finalized, the system then sends the command to the milling unit to make the part.

The milling unit has robotic arms that carve the prosthetic part dry or irrigated.

Versatility: the prostheses that can be made on the Ceramill Motion 2 milling machine are crowns, frameworks for dental prostheses, crowns, bridges, inlays, onlays, dental contact lenses, individual abutments and telescopic crowns.

Fixed dental prostheses are an excellent treatment for replacing missing teeth, combining aesthetics and chewing ability, taking advantage of your original teeth.

A fixed prosthesis is a type of dental prosthesis that cannot be removed by the wearer. It differs from dentures, for example, which can be removed whenever the person wants. Single tooth reconstructions

Also known as dental blocks, onlays or inlays, they serve to restore a tooth when the crown is partially compromised, usually by decay. The prosthetic piece covers only the decayed part of the dental crown. It is bonded with dental cement after the site has been cleaned and prepared.

There are different materials used to make fixed partial dentures, the most common being metal alloy – metalloceramics, resin, porcelain and 3D zirconia (considered the most aesthetic material).

Dental crowns – caps, pivots and jackets

A prosthesis that replaces the dental crown when it has completely decayed. In these cases, the root can still be maintained with root canal treatment or removed and replaced with a dental implant. The prosthetic dental crown is made of resin or porcelain.

When the root can still be maintained, the prosthetic crown is then bonded to the prepared tooth or to an abutment installed inside the treated canal. When an implant is indicated to replace the root, the prosthetic crown is screwed onto the implant.

Fixed Bridges – Fixed Dentures

Multiple crown prosthesis to fill a larger space. It can also be fixed on implants or on previously prepared teeth used as abutments. The indication depends on tooth and root decay.

Dentures for all teeth – full mouth / rehabilitations

It replaces the complete absence of teeth from the arches. For a long time, dentures were the only way to make up for this absence, but there are some known discomforts with this type of prosthesis, such as mobility due to the lack of complete adhesion and possible injuries.

The fixed full denture has come as a solution because it is completely stable in the mouth for the elderly, giving them the security to speak, chew and smile.

Fixed dental prostheses on dental implants can be made of resin (Bränemark protocol), porcelain with a metal internal structure or metal-free zirconia.

After having extensive dental prostheses, the patient can have an even better result with aesthetic harmonization treatments or lip fillings.

It is always important to remember that, once treatment has been completed, you should visit your dentist every 6 months for prophylaxis and to monitor the results of your treatment.

First of all, teeth are very important for health, aesthetics and well-being. The absence of teeth therefore compromises speech, chewing, social interaction and even general health.

In addition, difficulty chewing can sometimes influence the poor intake of important nutrients.

3D tooth printer

3D printers are very versatile, because they are able to build objects of various shapes according to a three-dimensional digital design. Prostheses and dental restorations are also being made using this concept at Clinica Dentaria ImplArt.

We have the two main 3D tooth printers developed in the world: Cerec from the German company Sirona (including the most up-to-date version of the system, Cerec Primemill), and Ceramill Motion II from Austria’s AmannGirrbach. They have robotic arms that mill the chosen dental material (which can be porcelain, ceramic, zirconia, etc.) until it acquires the shape of the computer-designed restoration.

The result is a piece made very quickly, completely customized to the patient’s needs, with perfect color and fit. It is also possible to make an identical copy of the shape of a prosthesis that the patient is already used to, but which needs to be replaced because it has suffered a fracture or was made with unsightly material (metal or stained resin).

Intraoral scanner

The intraoral scanner is a high-precision camera that captures intraoral images, i.e. of the teeth and gums. These images are transferred to a computer which creates a three-dimensional digital mold of the dental arches, the basis for planning the entire treatment.

With digital modeling, our team is able to design restorations and prostheses according to each patient’s needs, even mimicking the shades of neighboring teeth.

For the patient, this type of molding is much more comfortable and faster, unlike the old putty molds. Computer modeling also avoids errors in dental prostheses made with conventional putty molds.
