In some cases, it is not possible to place dental implants without first doing bone grafts. People who have lost one or more teeth due to accidents or extractions always think about reconstructing their dental arch in order to smile without embarrassment and for this, dental implant placement is certainly an excellent option.
When people lose their teeth and don’t replace them immediately, they experience bone resorption. As a result, the bone in the area becomes thinner and taller, or has defects in its shape. In the upper jaw, the size of the maxillary sinus can increase, which in principle prevents implants from being placed. In addition, with tooth loss, expressions become limited, the mouth shrivels, chewing becomes difficult and speech can also be altered.
In this way , bone grafting serves to rebuild enough structure to allow the implant to be made, restoring the lost volume to the atrophied bone. For this technique, the dentist can use the patient’s own bone material, taken from the mouth (for small reconstructions) or taken from other sites, such as the hip bone (iliac) and skull cap (for large extensions).
There are different bone grafting techniques, each with its own particularities
In the case of smaller, more common grafts, the bone grafting procedure can be done in the office using local anesthesia. For larger areas, the surgical intervention is performed in a hospital by a larger, multidisciplinary team under general anesthesia. The patient does not lose teeth while the bone graft heals.
It is also possible to use a xenogenous bone graft crystal, when the bone comes from animals (freeze-dried bovine bone – Genox or Bioss), human bone from bone banks (bone transplant) or synthetic vitreous materials (synthetic hydroxyapatite). However, the most easily regenerated material is bone graft taken from the person themselves (autogenous bone).

The procedure can be carried out in fragments or as a block. Many people are afraid of undergoing a bone graft, but in fact the procedure can be very simple. All the materials are biosecure and undergo strict controls to prevent contamination.

What are bone grafts?
Maxillary sinus lift or sinus lift
A sinus lift is a surgical procedure for bone grafting in the posterior region of the upper jaw. The aim is to increase the amount of bone in height so that a longer dental implant can be placed.
When a person loses one or more teeth in the upper arch, there is a loss of bone in height and the maxillary sinus shrinks (fig.1). The procedure is carried out in this way: First, a small opening is made in the gum in the lateral region of the bone defect (fig.2).
A variable amount of bone regeneration precursor material is then placed inside this opening (figs.3,4). The surgery is quick and usually takes no more than 30 minutes with an experienced surgeon. Bone regeneration takes between 4 and 9 months (fig.5), and then the implants are placed (fig.6), but radiographic examinations are carried out beforehand to check the progress of bone regeneration.

How does the All on 4 technique work? Is this technique right for me?
The All on 4 protocol consists of the surgical placement of 4 implant pins strategically distributed throughout the arch, followed by the fixing of a complete dental prosthesis.
The technique is generally indicated for people who have lost all their teeth in a full arch, upper or lower. Many of them wear mobiledentures and have advanced bone resorption(low bone volume), i.e. they have bone loss and are unable to place several dental implants in the arch.
We can therefore define the All on 4 technique as a dental implant alternative for people with little bone volume. Therefore, there is no need for bone grafts.
Another option for those with little bone volume is to use short or thinner implants, often avoiding bone grafting procedures.

The immediate loading technique can be defined as one in which the prosthesis is fixed more quickly after the implants have been placed. Most of the time, on the same day.
What is the difference between the All on four technique and the traditional protocol prosthesis technique?
In the conventional technique, 6 dental implant pins are generally used throughout the arch to support the protocol prosthesis.
With the All on four technique, four dental implant screws are used, the two posterior ones being longer. They are placed at a greater angle than conventional implants, around 45°. The longer length of the post is intentional to increase the contact area between the implant surface and the bone. This reduces the need to recreate a vertical bone augmentation through bone grafting surgery.

As well as increasing the contact area, the use of longer, tilted implants allows them to be placed in a stronger portion of the bone, so that they are securely attached.
Eventually, someone might ask whether this technique would be less efficient than the conventional one, imagining that the load distribution on 4 implants would be greater than 6 pins, for example.
Biomechanical studies show that tilted implants, when they are part of the prosthetic support, have no negative effect on load distribution. Implant tilting has been used in clinical practice for over a decade and has shown favorable results.
Advantages of total prosthesis on All-on-4 implants compared to dentures:
The dental prosthesis is stable because it is attached to the implants. This makes speaking and chewing more comfortable and secure. With a mobile prosthesis, the wearer may feel their prosthesis is unstable.
Dental implants are, in most cases, able to maintain bone volume. In contrast to implant-supported dentures, mobile dentures are unable to prevent bone loss and, as a result, the bone can become thinner and smaller. This is also one of the factors that makes dentures loose due to maladjustment.
The aesthetics of the prosthesis for rehabilitation in undoubtedly much better than a mobile prosthesis. Ceramics, applied zirconia and pure zirconia are advanced technology materials that more faithfully reproduce the characteristics of human teeth, for example, strength, shine, translucency, shades and textures, whereas mobile prostheses are made of acrylic or resin, materials that scratch, fracture and stain more easily.

Computer-guided implants for less invasive surgery and faster recovery
Rehabilitation with All on 4 implants is best performed using computerized planning, better known as guided implant surgery.
Due to the limited bone volume, we perform a CT scan to accurately measure the width and height of the available bone. Orofacial harmonization is a relatively new dental specialty, but it allows the specialized dental surgeon to perform aesthetic and functional procedures on the face and neck.
This specialty has emerged as an artifice to complement orofacial aesthetics, and always combined with dental treatment, it aims to create harmony with the patient’s smile and other facial features.
What is orofacial harmonization?

The surgery is performed by a dentist trained in this technique and consists of removing two fatty tissues located on either side of the face between the jaw and the mandible, called Bichat’s balls. First, an anesthetic is applied and then small incisions are made in the oral mucosa (inside of the mouth) to remove the Bichat’s balls with the help of tweezers. Finally, small sutures are made and the patient is given instructions on how to recover after the operation, which should be calm and with little or no pain.

The procedure is carried out simply and quickly by a dental surgeon qualified in orofacial harmonization. It is not capable of altering the natural features of the face, only correcting these imperfections. Superficial wrinkles can be treated with botulinum toxin (Botox).

How deoxycholic acid works for jowls

What is the chemical lipo procedure like?
Before starting the application, a topical anesthetic is used to eliminate any possible discomfort from the needle prick. We then apply deoxycholic acid in the form of a synthetic drug to the adipose tissue, distributed in small quantities over the entire length of the jowls.
In the first few days, swelling, bruising, pain (which can be controlled with painkillers), erythema (redness) and numbness may occur, but this is normal and is part of the fat elimination process. During this period it is recommended not to take anti-inflammatory drugs, as they prevent the product from working.
Up to six sessions are recommended to obtain a satisfactory result (depending on the degree of fat accumulation), with intervals of one month between each session.
In some cases, treatment can be enhanced with the use of a compression/microporous bandage at night after the fourth day of application, as well as local drainage to help reduce swelling and speed up the elimination of fat.
Duration of jowl treatment: the effect of the treatment does not have a set time, but it is expected that the accumulation of fat in this area will not return, except in cases of excessive weight gain.
Contraindications to deoxycholic acid for jowls: in principle, local infection, pregnant or breastfeeding women, people with cysts at the site of the jowls, goiters or thyroid nodules.
