Will I lose my dentures during dental bone grafting?

Before answering this question, it is necessary to explain that there are different dental bone grafting techniques, each with its own complexity and extent. This directly influences the guidelines for the post-operative period and recovery time.

In most cases it is possible to use a prosthesis while the graft heals

Dental bone grafting to allow the placement of just one implant, where the patient has teeth to be used as support for the prosthesis, is not the same as block grafting to place several implants in a full arch, which is considered more complex.

However, in many cases, it is possible to attach a temporary resin prosthesis, which is suitable for helping the tooth graft recover and heal, as well as allowing the patient not to be without teeth for the duration of the treatment.

After the recovery period, the temporary prosthesis is replaced by a permanent prosthesis made from the material chosen jointly by the patient and the dentist (resin, metal-ceramic or fully esthetic zirconia).

Types of tooth graft

Particulate Graft

Particulate grafting is used in small reconstructions, such as filling in a site where there has been bone loss or in the maxillary sinus lift technique. The biomaterials used in particulate bone grafting are implanted to replace or repair atrophied tissue.

They can be of natural origin (a small amount of crushed human bone from the patient themselves or from a tissue bank for transplants, or freeze-dried bovine bone powder).

The particulate graft can also be made of synthetic material produced in the laboratory, fully biocompatible with osteoconductive properties (attraction of bone tissue-forming cells) or synthetic vitreous materials (synthetic hydroxyapatite).

The particulate dental graft is deposited in situ and then covered with a collagen membrane. In favorable cases, the dental implant can be placed on the same day as the graft, followed by a temporary prosthesis.

Block Grafting

This technique is indicated for more extensive reconstructions. The block-shaped tooth graft can be taken from another part of the patient’s body. When only one block is needed, it can be taken from the mandible.

When more blocks are needed, they can be taken from the pelvis or skullcap. None of the donor sites suffer from bone loss. The blocks are fixed in the jawbone with titanium screws and will thus allow more dental implants to be placed.

Single tooth reconstructions

Also known as dental blocks, onlays or inlays, they serve to restore a tooth when the crown is partially compromised, usually by decay. The prosthetic piece covers only the decayed part of the dental crown. It is bonded with dental cement after the site has been cleaned and prepared.

There are different materials used to make fixed partial dentures, the most common being metal alloy – metalloceramics, resin, porcelain and 3D zirconia (considered the most aesthetic material).

Dental crowns – caps, pivots and jackets

A prosthesis that replaces the dental crown when it has completely decayed. In these cases, the root can still be maintained with root canal treatment or removed and replaced with a dental implant. The prosthetic dental crown is made of resin or porcelain.

When the root can still be maintained, the prosthetic crown is then bonded to the prepared tooth or to an abutment installed inside the treated canal. When an implant is indicated to replace the root, the prosthetic crown is screwed onto the implant.

Dentures for all teeth – full mouth / rehabilitations

It replaces the complete absence of teeth from the arches. For a long time, dentures were the only way to make up for this absence, but there are some known discomforts with this type of prosthesis, such as mobility due to the lack of complete adhesion and possible injuries.

Implanting several teeth at the same time

The loss of multiple teeth can certainly affect your ability to chew, as well as the beauty of your smile. The best option for replacing multiple teeth is certainly the placement of a multi-tooth implant, i.e. the placement of several implants and the attachment of a dental prosthesis. A multi-tooth implant can also be called a total implant or a protocol prosthesis.

What is periodontal scraping and what is it for?

Periodontal disease is an inflammatory disease that can affect the gum tissue and the bone around the teeth. There are three stages of the disease: gingivitis, periodontitis and advanced periodontitis.

It arises mainly from the accumulation of bacterial plaque and tartar on the teeth and if the disease is not controlled, over time it can lead to deterioration of the tissues that support the teeth. As a result, dental implants may be necessary due to tooth loss.

Red gums are a warning sign

Bacterial activity releases toxins that inflame the gums and cause redness and bleeding. If not initially controlled, the gums take on the shape of pockets (called periodontal pockets) and make room for plaque and tartar to form on the tooth root.

It is therefore at this point that periodontal scraping of this material becomes necessary to avoid more serious problems, such as the tooth softening and falling out.

However, periodontal scraping is a procedure that aims to mechanically remove tartar buildup at the level of the tooth root and helps to control and prevent the progression of periodontal disease.

Periodontal scraping and smoothing of the tooth and root removes the material that causes bacterial activity, allowing the gums to heal and return to their normal position. It is therefore done manually with the aid of specific instruments or ultrasonic vibration.

In most cases, the procedure is carried out under local anesthesia to avoid pain and discomfort for the patient, who usually already has sensitive and inflamed gums. It may be necessary to carry out more than one session in order to remove them completely.

It all depends on the stage of the disease. However, after treatment, patients are advised to take care of their oral hygiene to prevent the disease from reappearing and to return to the dentist’s office more often.

What is the best material for pure porcelain digital prostheses?

The digital porcelain crown is made as a monoblock, i.e. it has no internal reinforcement layers, as the computer will create the crown from a prefabricated block of dental porcelain.

The Pura Digital porcelain crown is an evolution in the materials used in dental prostheses. With it, it is possible to reproduce details that make the prosthesis similar to natural teeth. With the Cerec System, the e.max crown is made more quickly and precisely, because it is designed on a computer and sculpted by robots.

To make digitalpure porcelain crowns , we use CAD/CAM techniques:

1 – Intraoral scanner: It all starts with the digital molding of the dental implant with a high-precision color camera that takes images of the oral interior and sends them to the computer.

2 – Computer and software: receives the information captured by the intraoral scanner and carries out the simulation and personalized planning of the dental crown using three-dimensional images.

3 – Machine manufacturing: robots mill the e.max ceramic block based on the computerized design until the shape of the crown is achieved. The result is certainly a piece that is strictly compatible with the patient’s needs.

It is common for a temporary crown to be made before finalizing the permanent tooth to test whether the computer modeling is correct. Immediately after milling, the piece undergoes a crystallization process at approximately 850°C for 20 to 30 minutes and acquires high resistance, shade and translucency.

When the crown is finished, it is attached to the previously installed dental implant. For your convenience, the Cerec System and e.max ceramics are available at ImplArt Dental Clinic in Brazil. As an alternative to the pure porcelain crown, we now have an even more resistant material, translucent pure zirconia.
