Orofacial harmonization

Orofacial harmonization is a relatively new dental specialty, but it allows the specialized dental surgeon to perform aesthetic and functional procedures on the face and neck.

This specialty has emerged as an artifice to complement orofacial aesthetics, and always combined with dental treatment, it aims to create harmony with the patient’s smile and other facial features.

What is orofacial harmonization?

The surgery is performed by a dentist trained in this technique and consists of removing two fatty tissues located on either side of the face between the jaw and the mandible, called Bichat’s balls. First, an anesthetic is applied and then small incisions are made in the oral mucosa (inside of the mouth) to remove the Bichat’s balls with the help of tweezers. Finally, small sutures are made and the patient is given instructions on how to recover after the operation, which should be calm and with little or no pain.

Bichectomy risks: there may be asymmetry in the result, but this is rare and is more likely to happen if performed by professionals who are not trained in this technique.

What is the Chinese moustache filling procedure like?

How deoxycholic acid works for jowls

This helps to eliminate the broken down fat (a process that should take place over a maximum of 30 days).

What is the chemical lipo procedure like?

Before starting the application, a topical anesthetic is used to eliminate any possible discomfort from the needle prick. We then apply deoxycholic acid in the form of a synthetic drug to the adipose tissue, distributed in small quantities over the entire length of the jowls.

In the first few days, swelling, bruising, pain (which can be controlled with painkillers), erythema (redness) and numbness may occur, but this is normal and is part of the fat elimination process. During this period it is recommended not to take anti-inflammatory drugs, as they prevent the product from working.

Up to six sessions are recommended to obtain a satisfactory result (depending on the degree of fat accumulation), with intervals of one month between each session.

In some cases, treatment can be enhanced with the use of a compression/microporous bandage at night after the fourth day of application, as well as local drainage to help reduce swelling and speed up the elimination of fat.

Duration of jowl treatment: the effect of the treatment does not have a set time, but it is expected that the accumulation of fat in this area will not return, except in cases of excessive weight gain.

Contraindications to deoxycholic acid for jowls: in principle, local infection, pregnant or breastfeeding women, people with cysts at the site of the jowls, goiters or thyroid nodules.

Bruxism is a parafunctional disorder that causes people to grind their teeth or clench their teeth involuntarily during sleep. The excessive force of bruxism causes wear and tear on the teeth, pain in the muscles and bones (joints, jaw), as well as ringing in the ears and head and neck pain.

Are implants aesthetically pleasing?

In short, it depends a lot on the system used and the local conditions. Aesthetics have improved a lot in recent years. But it’s important to bear in mind that, however good the implant and the professional, the first is still a prosthesis on an implant, i.e. the replacement of natural teeth with artificial ones.

Therefore, expectations regarding implants are common, but can usually be followed by a certain amount of frustration. In many cases, the aesthetic solution is quite acceptable. Fortunately, there have been many advances, not only in the materials used to make prostheses, but also in techniques.

Nowadays we can build prostheses that look more and more like natural teeth. However, the main objective must always be functional. The implant is far superior to other prosthetic procedures, and in the absence of teeth, it’s the best we can do.

How long will I be out of implant prostheses?

In simpler cases, some kind of temporary implant prosthesis is provided for the patient to use immediately after surgery. In the case of the totally edentulous, the period is limited to a few hours to a few days after the first surgery. In the second stage, once the implants have healed, the patient is not left without the prosthesis. In some cases, the immediate loading technique can be used.

Can I eat anything after the prostheses have been fitted? What if I fracture a tooth in the implant-supported prosthesis, is the implant lost?

Not everything can be eaten, but the restrictions are not too severe. Certain foods can even fracture natural teeth. In short, a diet with a minimum of care is sufficient to preserve the teeth of implant-supported prostheses. One positive fact is that repairing fractured teeth is relatively easy.

What happens if the implant (post) becomes mobile after the prosthesis (tooth) has been placed?

All mobility is progressive and indicates that the implant has been lost. Each case is assessed individually and, in principle, the implant can almost always be replaced. This event brings frustration not only for the patient, but also for the dentist. In fact, what happens is an increase in treatment time and, above all, the need to be patient in order to complete it.

I have a full upper prosthesis. I would like to know about the possibility of placing an overdenture without a palate. How many implants? What are the costs?

The overdenture without a palate is a good option because it makes the wearer feel more comfortable. To do this, at least four implants need to be placed. Another option is a fixed prosthesis on implants. This would require four to six implants.

The cost of this type of treatment is variable and it depends on tests to predict the availability of bone in the area. In other words, prices can only be given after a face-to-face consultation and an assessment of the particularities of each case.

I would like to know about the possibility of replacing two removable prostheses with staples with fixed prostheses without staples or implants. I would also like to eliminate the visualization of the staples in my smile.

A treatment like this appears to be of little complexity and can be completed in a short time, according to our Day Clinic philosophy. We always suggest that you also have your teeth whitened to make your whole smile whiter and more aesthetically pleasing.

I have two fixed bridges (one on each side) on the top and I have two implants started (the pins have been placed). I’d like to complete the implants and replace all my teeth with nice white ones, because mine are yellow and the bridges are old with the metal showing. I would also like to know how many days it would take to do a treatment.

Should I go back to the dentist after having my teeth fitted?

At least one clinical radiographic check-up is required every year. It is also the patient’s obligation to attend these check-ups. Appointments are important to identify any problems in time for them to be reversed. They are also used to carry out professional cleaning of the prosthesis, to assess whether the user is performing daily oral hygiene properly, among other aspects that only a professional dentist can identify.

Will chewing ability improve after implants?

Above all, implants have much better functional results than dentures and removable prostheses. Patients who have been wearing dentures for a long time and have implants fitted feel the difference is very significant. The fixed implant prosthesis does not move and the patient certainly feels more comfortable and secure when chewing, speaking and smiling.

I’ve heard that porcelain is the best material for teeth. If I want an implant-supported prosthesis with porcelain teeth, would that be possible?

In most cases it is possible to make a prosthesis on an implant with porcelain teeth. Porcelain teeth are undoubtedly the best material for making a dental prosthesis. Today there are porcelain teeth with or without internal metal that offer different aesthetic results, with metal-free porcelain being more aesthetic than prostheses containing metal.

Gum grafting to improve the aesthetics of teeth

What is gum grafting?

Gum grafting is used to try and reconstruct an area of the mouth that has suffered from receding or lost gum tissue. Gum recession can occur due, for example, to brushing too hard, gum disease, bruxism or prolonged use of braces. To reduce the effect of the problem, gum grafting is usually indicated.

This is a surgical procedure that consists of removing a piece of mucosa from the patient’s own palate (roof of the mouth) and then placing it over the defect. The material is removed under local anesthesia and the person feels no pain, only slight discomfort.

Update: It is now possible to perform gum grafting without removing gum from the roof of the mouth, using a synthetic material for this purpose.

When healthy, the gums form a protective collar around the tooth. There are two types of gum: the inserted gum, which is a pink, thicker tissue that surrounds the tooth and is adhered to by the bone tissue, and the mucosa (non-inserted gum), a thin, soft and highly mobile tissue that forms the inside of the cheeks and lips. When the gums are unhealthy, we first need to restore their health by removing the bacteria with a periodontal scraping.

Gum defect needs grafting

A gingival defect (receding gums) usually occurs as a result of gum inflammation, caused by a tooth fracture for example. It happens when the gum tissue retracts, almost always due to excessive force when brushing the teeth. The retraction of the gum tissue leaves the tooth root exposed, causing tooth sensitivity and compromising the aesthetics of the teeth. In the case of dental implants, receding gums can expose the base of the implants.

There can also be gum loss due to trauma, accidents or periodontal disease. In these cases, the gums lose their original shape and the aesthetics of the smile can be compromised, especially in the area of the front teeth. In some cases, the placement of dental implants leaves the gums with an inappropriate shape, and this defect can also be corrected with the gum grafting technique.

It is important to note that the gingival grafting procedure does not guarantee success. Even if the procedure is carried out perfectly in terms of technique, there may be tissue necrosis and a consequent loss of the grafted tissue.

To prevent pain, the patient uses medication (anti-inflammatories) before and after surgery. The dentist can also use “surgical cement”, a protective barrier that is fixed in place where the operation took place. It sticks to the teeth and gums for around five days.
