This technique eliminates the need to make large cuts and sutures in the gums, making the post-operative period easier and the recovery faster.
The surgical guide acts as a template with indications of where the incisions should be made to place the dental implants. The surgical guide is custom-made according to the patient’s CT scan images, using a 3D printing method.
CT makes it possible to analyze the bone volume of all regions of the maxilla and mandible in order to select the best distribution of implants. In this way, it is also possible to choose the most suitable implants, taking length and diameter into account.
Another advantage of computer-guided implant surgery is that it helps to avoid surgical accidents, such as hitting delicate structures like nerve pathways.

The patient can leave the office with temporary teeth or with the mobile prosthesis they were already wearing over the implants until the definitive prosthesis is finished (in 3 to 6 months), or with an immediate load in up to 72 hours.
Immediate loading ranges from the guided implant surgery for placing the implants to the fixing of the fixed dental prosthesis, usually of the dental protocol type. The patient can quickly speak, smile and eat safely (the masticatory effort should progress little by little, according to the surgeon’s instructions).
Indications for a computer-guided implant surgery:
Any completely edentulous patient, whether recent or having worndentures for years, who has good bone availability is likely to be a good candidate for this technique.
Diabetic and/or hypertensive patients who generally have no indication for surgery or when it should be performed with caution due to bleeding. Computer-guided total implant placement surgery is, in principle, an excellent option for this patient, as bleeding is less with this technique.
– Very advanced bone atrophy makes it impossible, in principle, to place neodent dental implants. In some cases, this problem can be solved with another technique, bone grafting.

Single-tooth implants with immediate loading consist of placing one or a few dental implants and then attaching a fixed prosthesis. This technique is used to reduce the stages of the procedure and allow the smile to be restored more quickly with implants. The tooth is fixed directly to the implant during surgery or within a few days.
The single-tooth implant immediate loading technique makes your treatment faster
In a common procedure, a temporary acrylic tooth is placed in the correct position for a period of one to four months (healing period), depending on the type of implant chosen. Soon after this period, the implant and bone are closer together and the gums are more stable, making it possible to make the molds for the permanent porcelain tooth.
In the case of immediate loading for single implants, the technique is used to rehabilitate just one tooth. This type of intervention is used when the tooth is severely compromised by cavities, periodontal disease, fractures or simply the absence of a tooth (a tooth that hasn’tbeen born).
The extraction of a problem tooth is followed by the installation of the implant and the fixing of an immediate prosthesis by the implant dentist. Even with planning, in some cases there are uncertainties about the use of immediate loading.Some factors cannot be predicted, such as the stability of the implant (insertion torque) and the quality of the bone.
The technique is particularly suitable for the front teeth, especially the incisors, which require less chewing force. The bottom teeth are not suitable for immediate loading because chewing exerts too much force. Therefore, immediate loading in this area reduces the chances of successful treatment.
To carry out the single implant immediate loading procedure, the patient must be in good health with good oral conditions to facilitate the primary stability of the implant. In addition, favorable bone structure is essential for immediate loading.
Uncontrolled systemic diseases and smoking are factors that should be assessed by the dentist, as smoking reduces the chances of osseointegration.

Scientific History Dr. Roberto Markarian
Dr. Roberto Markarian graduated as a dental surgeon in 2001 from the USP School of Dentistry in São Paulo. While still a student, he carried out research into improving dental materials, particularly in the areas of dentistry, prosthodontics and implantology. As aresult of this work, he received 5 awards in Brazil. He described his results in more than 30 scientific articles, which were later published in international journals.
In the meantime, he has taken postgraduate courses in prosthetics and dental implants and has become a researcher and assistant to the fixed prosthetics team at USP (São Paulo)and at the São Leopoldo Mandic College (Campinas), as well as teaching college students and dentists.
In the area of academic research, Dr. Roberto Markarian has been conducting courses and research in the area of implant dentistry since 1998, and in the area of digital dentistry since 2004. He also gives lectures and training courses on modern digital dentistry tools.

The term is related to pressure, i.e. loading on the newly installed implant, therefore immediately after surgery. It is important to note that the decision on whether or not to perform immediate loading on the implant should be made by the implant dentist. Assuming that this is the main desire of all patients, it is the implant dentist who must certainly assess the conditions conducive to the use of this technique.
Currently, implant surgery is studied beforehand by the implant dentist (dental implant specialist) through the patient’s panoramic X-ray and CT scan. It is therefore possible to assess whether the patient has favorable bone conditions for the use of this technique. However, it is at the time of implant surgery, by locking the implant, that it is possible to guarantee that the immediate loading technique can be carried out. In this way, the professional can also change the technique used to make the prosthesis if the necessary implants are not locked during surgery. Although the bone structure is studied beforehand through examinations, it is only during surgery that the real bone situation can be gauged.
How is immediate loading surgery with dental implants performed?
Surgery is performed to install dental implants in the jawbone. The surgery should take place like a conventional dental implant surgery. If the surgery goes well, you can then start on the provisional or even definitive prosthesis, which requires molds and measurements of the arch. Immediate loading is usually carried out with a temporary prosthesis and, after the implants and gums have healed (between 3 and 6 months), the definitive prosthesis is made. Although it is possible to carry out the treatment with the definitive material initially, we consider it better to make a temporary prosthesis, as the gums must swell and the bone changes shape during the initial three months of healing after surgery.

The technique is not always used in all cases, as it depends on the patient having some specific and favorable conditions that are assessed in consultation.
